Indiana-owned home care provider Advantage Home Health Care has disclosed a server breach that exposed the personal information of 19,851 patients and current or former employees. According to the company, an unauthorized party accessed one of its servers on June 9.
Advantage Home Health Care discovered the incident on June 16 and subsequently confirmed on June 26 that the intruder extracted files potentially containing personal data. The breach was formally reported to the federal breach list on August 25 as a network server hacking attack.
Compromised files for patients may include names, dates of birth, Social Security numbers, home addresses, phone numbers, and details regarding medical conditions and care. For certain employees, the exposed records may also feature job histories and health plan information.
In response to the incident, the company notified federal law enforcement, engaged forensic specialists, and began mailing notification letters around August 25. Advantage Home Health Care is offering affected individuals 12 months of free credit monitoring, a credit report, a credit score, and cyber monitoring services through Cyberscout. Individuals who received a notification letter can contact 1-888-398-2084 for assistance.
The company stated that it is currently unaware of any instances of fraud connected to the exposed files. Advantage Home Health Care operates nine locations across 46 counties in Indiana.
